Tryout Info & Forms

Additional  sport tryout information for each sport is located under the sports information tab.

You must have a physical and concussion form on file prior to try-outs.   If physical was completed after January 1, 2016 then it is good for 395 Days.  If completed prior to January 1, 2016 it is expired. 

In this section you will find the current Athletic Participation Form (physical form) and the Concussion Form. According to new federal legislation, Gfeller-Waller Law, both athlete and parent/guardion must fill out the concussion form prior to trying out for a sport.  

The concussion forms must be signed by the student and the parent before a child can participate.

Both forms must be completely filled out and returned prior to trying out for an athletic team. All forms are within the same form now.

The participation form must be filled out by a doctor prior to try-outs.

Eligibility requirements are also listed in this area. Wake County is responsible for eligibility requirements. The school is unable to change any regulations in regard to eligibility.
