The Covid-19 Pre-Participation Health Questionnaire must be printed, filled out, and signed by both parent and student. The form must be turned in on the first day of workouts.

- All WCPSS Middle Schools will be holding sport specific workouts in-lieu of athletic competitions this year. The workout seasons are as follows.
Fall Sports (football, cheer, girls' soccer, volleyball) - Feb 22nd through March 19th
*Athletes must complete the Salem Middle School Athletic Registration linked below as well - Registration will include Athletic Participation Forms that will be submitted digitally EXCEPT for page 5, which will be turned in to Coach Honeycutt in person*
Spring Sports (Softball, Boys' and Girls' Track and Field, Boys' Soccer) - March 22nd through April 30th
Winter Sports (Boys' and Girls' Basketball, Cheer) - May 3rd through May 21st
Information for Spring and Winter sports will be coming out closer to when those seasons begin
7th and 8th grade students will need to sign up before the deadline for each season in order to participate and will only be able to participate on the days their cohort is in session. VA students will be assigned to participate with a specific cohort once all sign-ups are complete. There will be NO restrictions on participation related to grades or attendance. As long as all of the participation paperwork is completed, students will be eligible!
